- Assessment of 15 demerits for entraining onto equipment without communication with the Engineer that they were on the equipment and riding, while working as a Conductor on Train 235-04 on June 4, 2020.

- Assessment of 20 demerits for incorrectly lining three switches in London Yard, while working on Train 141-13 on June 14, 2020.

- Assessment of 30 day suspension for detraining a stationary loaded bulkhead flat and failed to maintain 3 points of contact, while working as a Conductor on Train 140-16.

- Assessment of 30 demerits/dismissal for a missed call when properly ordered for Train 141-02 at 0540 hrs. on August 3, 2020 – Violation of Information Bulletin No. SI-003-20 T&E Availability Standard dated January 21, 2020.

- Dismissal for accumulation of over 60 demerits as well as for having seven disciplinary infractions in a 12-month period, as per the Company’s Hybrid Discipline Guidelines. (COUNSEL)
